1. Overview

These Terms of Service govern your use of elementalivtherapy.com and any services you book with Elemental IV Therapy, LLC ("Elemental," "we," "us," or "our"). By using our website, placing an order, or receiving a service, you agree to these Terms. Our Privacy Policy, Refund & Cancellation Policy, and Service Area & Appointments policy are part of these Terms.

2. Our services

Elemental provides mobile IV hydration, NAD+ infusions, and vitamin injections. Every service is administered in person by a licensed registered nurse at a location you choose. Our services are wellness-oriented. They are not intended to diagnose, treat, cure, or prevent any disease, and they are not a substitute for care from your own physician. For a medical emergency, call 911.

3. Medical oversight

All services follow protocols approved and overseen by a licensed physician serving as our Medical Director, with clinical oversight from a licensed nurse practitioner. Nurses administer services only under those protocols and only after you are cleared by a licensed provider.

4. Health screening and clearance

Every client must complete a health intake and a telehealth screening (a good faith examination) with a licensed provider before receiving any service. Clearance is decided solely by the reviewing provider and is not guaranteed. A purchase does not guarantee treatment.

  • If you are not cleared, you receive a full refund for that service.
  • Your nurse may also decline, pause, or stop a service at the visit if your vital signs, symptoms, or setting raise a safety concern.
  • You agree to give complete and accurate information about your health history, medications, supplements, allergies, and whether you are pregnant or breastfeeding, and to tell us about any change before each visit.

5. Informed consent

Before your first service, you will review and sign consent forms through our secure medical records system. These explain the service, its expected benefits, and its risks, which may include bruising, discomfort, infiltration, phlebitis, lightheadedness, and in rare cases allergic reaction or infection. You may ask questions at any time and may withdraw consent before or during a service.

6. Eligibility

You must be at least 18 years old to book or receive a service. By booking, you confirm that you, and every guest you book for, are 18 or older. We may ask for photo ID at the visit. If you book for other people, you confirm they have agreed to these Terms and will complete their own screening and consent.

7. Service area and appointments

We travel to homes, hotels, offices, and yachts in South Florida, including Aventura, Bal Harbour, Surfside, Sunny Isles Beach, Golden Beach, North Miami, North Miami Beach, Brickell, Miami, Hallandale Beach, Hollywood, and Fort Lauderdale. Locations outside this area are at our discretion and may carry a travel fee, which we confirm with you before the visit. You agree to provide a safe, clean, accessible space for your nurse and accurate access instructions (gate codes, building, concierge, or marina details).

8. Booking, payment, and cancellations

Prices are listed in US dollars. A deposit is charged when you book, and the remaining balance is due at your visit. Your appointment time is confirmed by text after you book.

  • You may cancel or reschedule free of charge up to 12 hours before your appointment.
  • If you cancel less than 12 hours before your appointment, or are unavailable when your nurse arrives, your deposit is non-refundable.
  • If a provider does not clear you for treatment, your deposit is refunded in full.

9. After your visit

Contact us right away about any unexpected reaction after a service, and seek emergency care or call 911 for any serious symptom. Follow any aftercare guidance your nurse gives you.

10. Limitation of liability

To the fullest extent permitted by law, Elemental and its staff are not liable for indirect, incidental, special, or consequential damages arising from your use of our website or services. Our total liability for any claim related to a service will not exceed the amount you paid for that service. We are not responsible for outcomes caused by inaccurate or incomplete health information you provide. Nothing in these Terms limits liability that cannot be limited under Florida law.

11. Website use and content

Content on this website is for general information only and is not medical advice. All text, images, branding, and formulation names belong to Elemental IV Therapy, LLC and may not be copied without our written permission.

12. Governing law

These Terms are governed by the laws of the State of Florida. Any dispute will be resolved in the state or federal courts located in Miami-Dade County, Florida.

13. Changes to these Terms

We may update these Terms from time to time. The updated version takes effect when posted on this page.
